Notes:

*RotaTeq vaccine is an optional vaccine that must be started at the 2 month visit with a booster given at 4 month visit.

**Gardasil is an optional vaccine for females starting at age 11 to prevent cervical cancer and HPV. Gardasil is now recommended by the AAP for males as well to prevent oral and anal cancers caused by HPV.

***Pediarix is a vaccine that contains DtaP, Polio, and Hepatitis B.

****The Menactra booster is a new recommendation for 2011.

!!! EFFECTIVE IMMEDIATELY 2/4/15. Because of our Measles crisis in the U.S. we will switch the scheduling of the Varivax and MMR vaccines. The first dose of MMR will now be given at 12 months and the second dose of MMR will be given at the 4 year well check up. NO EXCEPTIONS. This is a public health issue and we are committed to protecting not only our patients but also those who cannot be immunized because of significant and compromising medical ailments. !!! This schedule may differ from what is printed in your current yellow book.

We recommend yearly check-ups after the age of 3.
